- comments: observed hand washing sink with no hot nor cold running water at front bar/prep area. Water lines are not connected and faucet is wrapped with masking tape. - comments: observed no 90-day shellfish tags with two 5lbs. Seperate pans of raw oysters inside of prep cooler and walk in cooler. Product was discarded. Must keep tag with product until finished or last one sold,date and maintain. Appx. 10lbs. $120. (priority foundation 7-38-005)(cos)

- comments: observed food containers stored on floor inside of walk in cooler & freezer. Must elevate off of floor and store properly to prevent cross-contamination.

- comments: must invert utensils stored on shelving units throughout prep area.

- comments: observed male food handler handling ready to eat foods with bare hands instructed to provide utensils,deli paper or gloves.

- comments: must clean grease and food debris build up from all prep cooler,walk in cooler shelving units,storage shelves,dish racks,grill table. Delime dish machine.

- comments: must repair or replace leaky pipe at hand sink and 3- compartment sink in salsa prep area.

- comments: observed a massive amount of empty boxes on stair case and floor in rear area. Must remove and discard properly daily to prevent harborage area for rodents.

- comments: must repair or replace stained ceiling tiles in women's customer's and dining area. Continue to grout new floor throughout rear prep/dish washing areas.

- comments: must repair or replace damaged filters at hood of main kitchen prep area.

- comments: previous core violations from report #2555219 5/4/22 not corrected #10- observed no hand washing signs at hand sinks in all prep,toilet rooms and bar areas. Must provide and maintain. #37-must label food storage containers when food is not in original package. #41-must store wiping cloths in a sanitizing solution when not in use. #47- must repair or replace worn door gaskets on 1- door reach in cooler & 2 door seafood prep cooler and missing interior panel of 2- door main line prep cooler. #49- must clean food debris build up from steam tables,fryer cabinets,walk in cooler-fanguard covers,prep tables. #60 observed no lids on garbage cans in women's toilet rooms. Must provide and maintain. #57-inspector comments: observed several expired food handlers training for employees. Must provide and maintain. #58-observed no food allergen training for food manager.
