- comments: observed no hand washing signs at hand washing sinks in washroom and dish washing areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed several bags of chips(jay plain, cheddar and sour cream, etc)torn, gnawed, adulterated with mice droppings and displayed for sale on shelving units. Instructed manager to discard. All food packaging shall be intact, protected from contamination and safe for human consumption. Priority foundation 7-38-005 citation issued

- comments: observed over 100 mice droppings scattered on floor under chip display shelves, inside cardboard trays on chip display shelves, on floor along walls and corners in sales and storage areas, on shelving units near electrical equipment in dish washing area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed 1/4" gap on bottom of rear door in west storage area. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed no covered waste receptacle in washroom. Instructed manager to provide.

- comments: observed both outside garbage dumpsters not covered, left lid missing on left dumpster. Instructed manager to replace missing lid and cover all dumpsters. Outside garbage dumpsters shall be covered with tight fitting lids. Priority foundation 7-38-020(b) citation issued

- comments: observed accumulated trash and debris on ground around dumpsters. Instructed manager to clean and maintain.
