- comments: observed no written employee health policy on the premises. Instructed management to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010,no citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005,no citation issued.

- comments: instructed to provide and maintain handwashing signage at all handsinks (kitchen,washrooms).

- comments: observed an open package of raw pork chops stored directly above an open container of cooked rice inside the kitchen 2 door stand-up cooler. Instructed manager to always store cooked and ready-to-eat foods above the uncooked (raw) foods. Foods must also be covered and protected to avoid cross-contamination. Priority violation 7-38-005,citation issued. Corrected on site(cos). - cos

- comments: instructed manager to provide and maintain a visible thermometer inside the kitchen 2 door prep cooler directly across the cooking stoves.

- comments: instructed manager to properly label and maintain bulk food containers/bins identified with common name (salt, sugar, flour etc).

- comments: observed gap/opening all around the exhaust fan above the rear exit door. Must pest proof exhaust fan with screen to prevent pest-entry.

- comments: observed approx:50 rodent(mice)droppings on the floor behind cluttered front counter and inside cabinets also, in the rear cluttered hot water tank room. Instructed to detail clean and remove all rodent droppings and sanitize all affected areas;additional pest control service is now reccomended. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed manager with no hair restraint in the food prep area. Instructed manager to provided hair restraints for all employees in the food prep area.

- comments: instructed manager to properly store all washcloths in a sanitizing solution especially when not in use.

- comments: observed torn door gaskets inside of the 2 door prep cooler directly across the cooking stoves. Instructed management to replace the torn door gaskets.

- comments: observed frozen foods stored in plastic grocery bags inside of the freezers. Instructed management to use only food grade storage bags or containers.

- comments: instructed to detail clean the exterior of grease trap,kitchen storage shelfs,prep tables, food storage containers, interior and exterior of all coolers,microwaves and deep fryer with grease and dried food accumulations.

- comments: observed water leak at drain pipe underneath the far right compartment of the 3-compartment sink. Instructed manager to repair plumbing leak and maintain.

- comments: instructed to detail clean floors with grease and dried food debris accumulation along the walls and in all corners throughout kitchen prep area and in the rear dry storage room also,detail clean and sanitize walls near the cooking equipments to remove food splashed debris and maintain.

- comments: instructed to detail clean and organize the front counter area and rear utility mop sink/hot water tank room and remove unnecessary articles/broken unused refrigerator in rear dry storage room due to potential pest harborage,relocate in use refrigerator stored in hot water tank room also,elevate stock items mops,brooms cleaning equipments off the floor at least 6' inches.

- comments: instructed to replace burnt-out light bulb at kitchen exhaust hood and inside kitchen 2 door stand-up cooler and maintain.

- comments: observed no food handler certificates provided on the premises. Must show proof of training for all food handling employees.

- comments: previous core violations not corrected from inspetion report# 2312997 dated:09/26/2019 #55-observed floors dirty under hot line equipment. Instructed management to detail clean floors under hot line equipment. Core violation dec 26, 2019 #58-observed no allergen certification on site. Instructed manaement to become allergen certified. Core violation dec 26, 2019 instructed to correct noted violations and maintain by 3/26/2021. Priority foundation violation 7-42-090,no citation issued.
