10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-204. 11: no handwashing sink was installed in the prep area by the quick-chill/walk-in coolers area. Observed one prep table within used cutting board and used knife on the yellow cutting board; diced potatoes in plastic container by the said board. Prep table located in front of walk-in cooler. Also observed at north wall, top of cooler unit used for food preparation. Blue cutting board with used knife on top said board, also observed three containers of chopped food: cabbage, yellow squash, and onions by the said board. Priority foundation violation: 7-38-020(c), no citation issued

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: 4-101. 19: both staff washrooms have toilet paper holders broken. Instructed to replace. Must not use milk crates as a means of elevation for stored items and used for seating. Provide raised shelves and chairs.

- comments: 5-501. 17: provide cover for receptacle inside the women's staff washroom(rear area).

54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 113(b): observed outside covering receptacles in poor condition: one lid is pushed toward inside the container, leaving a large gap, another container broken lid with half missing. Observed excess insects on garbage. Must provide new covers for mentioned receptacles to minimize harbor for insects and pest as well as create a public health nuisance. Lids must be tightly fitted to receptacles. Priority foundation violation:7-38-020(b), citation issued.

- comments: 6-201. 11 broken wall at dishroom (east area between walls and ceiling), above handwashing sink. Pieces of bricks inside the handwashing sink, fragments from walls, and wall residue on paper towels dispenser and top low temp dishmachine. Repair issue. Surface must be smooth, cleanable and non-absorbent surface.

- comments: 6-201. 11 broken floor tiles in the prep area, in different locations: in front of soup kettles, by the prep tables, etc. The surface must be smooth and cleanable. Repair broken walls inside the women's washroom, east wall.

- comments: 6-304. 11 no mechanical ventilation is provided in both staff washrooms. Must be provided.

- comments: 2-102. 13 expired allergen certificate posted on site. Must renew, also provide said certificate for the other manager with city sanitation certificate holder.
