- comments: no city of chicago certified manager on site while handling tcs foods (slicing and packaging raw meats), nor valid certificate posted. Instd to provide manager and valid certificate at all times. Citation issued 7-38-012 priority foundation

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no employee health policy on site. Must provide written employee acknowledged health policy for all employees. Priority foundation violation. 7-38-010 citation issued

5. Procedures for responding to vomiting and diarrheal events
- comments: no vomit and diarrhea clean up kit/procedure on site. Must provide written procedure for the clean up of vomit and diarrheal events, and corresponding equipment outline in procedure. Priority foundation violation 7-38-005 citation issued

- comments: display case unable to maintain proper cold holding temperature, air temperature at 56. 0f. Case tagged and held for inspection. Instd to not use until repaired and reinspected by cdph. Citation issued priority 7-38-005

- comments: over 100 mice droppings scattered on floor in electrical closet and on shelves and along wall base in rear storage area. Instd to remove all droppings, clean and sanitize all affected areas, and contact pest control for service 7-38-020(a) priority foundation citation issued

- comments: front entrance doors and rear exit doors not rodent proof, approx 1/4 inch gap inbetween and at bottom of doors. Instd to rodent proof same so as to be tight fitting.
