3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand signage at the hand sink in the employee/public women toliet room. Instructed to provide and maintain.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed low temperature dish washing machine in use not properly sanitizing. Sanitizing strength of sanitizer (chlorine) at the time of inspection was 0 ppm. Equipment must properly sanitize at a minimum of 50 ppm. Instructed not to use unit until reinspected by chicago department of health (cdph). Wash, rinse and sanitize all dishes in the three compartment sink. Tagged unit. Priority violation #7-38-025. Citation issued.

28. Toxic substances properly identified, stored, & used
- comments: observed cans of pesticide, bottles of toxic chemical agents stored of the dry storage shelf with tortilla shells and bulk containers of powders. Informed manager that toxic chemicals may not be used to store with food consumed by humans. Manager corrected violation during inspection. Priority violation # 7-38-005 citation issued.

36. Thermometers provided & accurate
- comments: observed no metal stem thermometer on site to monitor the internal food temperature of tcs foodsn (rice, shrimp, pork) instructed that the facility must obtain a metal stem thermometer and maintain proper thermometers on site. Priority foundation violation #7-38-005. Citation issued

37. Food properly labeled; original container
- comments: observed no labels for storage containers (salt and sugar). Instructed person in charge all bulk and working food ingredient containers must be labeled with contents name.

38. Insects, rodents, & animals not present
- comments: observed a 1/4' gap at the bottom of the rear exit door located in the basment. Must provide a tight fitting seal at all outer openings to prevent the entry of pests.

Observed approximately 200 rat droppings on the floor behind the sushi bar area and throughout the basement near the exit door, on the floors, corners and concrete ledges, boiler room/storage room throughout the basement, underneath strairs of the basment access door ,bathrooms, under prep coolers in ktchen. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority #7-38-020(a). Citation issued.

- comments: observed boxes of food stored on the floor in the walk-in cooler. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed noodles and vegetables stored in plastic grocery bags in the prep cooler and walk-in cooler. Management instructed to use proper food grade storage bags and containers only.

- comments: observed no chlorine chemical test kit available during inspection. Instructed manager to provide test kit for proper concentration in sanitizing solutions. Priority foundation 7-38-005 citation issued.

- comments: observed standing water, food and debris at the bottom of serveral reach in coolers located in the kitchen. Also, observed vegetables under storage shelf adjacent to rear door. Instructed to detail clean and maintain.

- comments: observed no covered receptacle in the women employee/public toliet room. Instrcuted to provide and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed missing and damaged floor tiles in the rear food prep/dish area. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times. Observed mutiple holes in the walls (ceiling near dish machine/rear food prep, basement ceiling) throughout the facility. Instructed to seal.

55. Physical facilities installed, maintained & clean
- comments: observed excessive clutter in the basement and rear area near the exit door. Instructed to organize and stored things at least 6 inches off the floor to prevent harborage for pest.

Instructed manager to clean floors under all equipment, along walls and in all corners in all areas. Observed dust buildup and splash inside light shields in cooking area and prep areas. Instructed manager to clean and maintain all fans, light shields and ceiling/wall vents.

- comments: observed no proof of training for all food handlers on site. Instructed to provide and maintain records at all times.

- Chicken Yakisoba (Wheat, Molluscs - Oysters) / Unitary

All batches of these products are included in the recall if their labels do not mention the presence of the allergens. These items were sold packaged in bowl-like cardboard containers or wrapped in clear plastic wrap and were sold refrigerated.

People with an allergy to wheat, eggs, shellfish or mustard, or those with celiac disease or another illness induced by the ingestion of gluten are advised not to consume the recalled products. They must return it to the establishment where they bought it.

In case you experience harm from allergens or undeclared ingredients, it is important to report it. It can help to detect & resolve issues and prevent others from being harmed, and it enables better surveillance. If symptoms persist, seek medical care.

I was close to going to the hospital or using an epi pen. It took almost three days to go down. I wasn't sure it was the pretzels at first. But a few days after I was doing better I had more pretzels and it happened again after I started eating them. I stopped again. I tried another brand and had no problems at all from the other brand.

There has to be too much of some ingredients in these or something that shouldn't be in them that has made its way in the product.

One thing I'd suggest the manufacturer and FDA/Regulators double check is the possible amount of cellulose containing ingredients if there is improperly processed cellulose or if it is being sourced from high risk for allergy wood and other sources. Perhaps also a pesticide issue with the wheat or other items used or improper cleaning of certain grains, etc. Possible mold contaminants may be present as well which are some of the worst allergies I suffer from. Based on my own personal allergies, there are a few possible culprits where things could have gone wrong and caused contamination at some point of the supply chain. I'm happy to help narrow it down and provide whatever I can to assist.
