3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees. Priority foundation violation #7-38-010. No citation issued.

23. Proper date marking and disposition
- comments: observed ready-to-eat, time/temperature control for safety (tcs) foods (cooked rice and chicken) not date marked to indicate the product's name and date in which the food must be sold, discarded or consumed within 7 days. Instructed to provide proper labels for date marking and discard date for refrigerated, ready-to-eat, tcs foods held over 24hrs placed in all cold-holding units. The chicken and rice was voluntarily discarded and disposed of 10 lbs of products worth $100. 00 through denaturing process. Cos priority foundation violation #7-38-005. Citation issued.

- comments: obsedrved no labels for bulk sugar, salt, and flour storage container stored in the rear near the walk-in coolers. Instructed person in charge all bulk and working food ingredient containers must be labeled with contents name.

38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on the premises. Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection report. Priority foundation 7-38-020(c). No citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates being used as shelving throughout thewalk-in freezer. Must remove and provide shelving that is 6' off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed frozen chicken, bread, pork, vegetables stored in plastic grocery bags in the walk-in freezer and one door freezer. Management instructed to use proper food grade storage bags and containers only.

- comments: observed a leak at the faucet of the three compartment sink. Instructed to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in the public women toliet room and employee restroom loctaed near the office. Instructed to provide.

57. All food employees have food handler training
- comments: observed expired food handler certificates. Instructed to maintain records at all times.

58. Allergen training as required
- comments: observed the certified food manager without an allergen training certificate. Management instructed to have all certified food managers obtain this certificate.
