10. Adequate handwashing sinks properly supplied and accessible
- comments: observed broken paper towel dispenser at hand washing sink located at entrance to food prep area. Instructed to repair and maintain in good condition. Priority foundation violation 7-38-030(c). No citation issued.

38. Insects, rodents, & animals not present
- comments: observed 10 live roaches behind hot line in food prep area, dish area, dry storage area, and service area. Additionally observed 2 live fruit flies in women's toilet room, and 8 live fruit flies on wine bottles mounted to wall near bar. Furthermore observed 50 mouse droppings in cabinet of coffee area, 20 mouse droppings in office, and 10 mouse droppings on storage shelves of storage room next to coffee area. Instructed to receive additional pest control service and maintain facility free of pest activity. Priority foundation 7-38-020(a). No citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed build up of food debris inside meat cutter in food prep area. Instructed to deep clean and maintain in good condition.

- comments: observed box freezer containing ice cream covered in heavy build up of ice. Instructed to defrost and maintain in good condition.

- comments: detail clean the cooking equipment interior and exterior throughout. Observed encrusted food debris and grease inside of all of the convection ovens and on the stoves, steam table, deep fryers, prep tables, storage shelves, storage cabinets, interior of prep coolers. Must clean all cooking equipment and food storage coolers. Clean the interior surfaces on the ice machine and bar equipment including the soda gun nozzles.

- comments: observed holes and gaps in walls and baseboards beneath walk-in coolers, in dish area, throughout food prep area, in service areas, and in dry storage area. Instructed all holes and gaps in physical facilities should be sealed to prevent harborage of pests.

- comments: : clean the encrusted grease on the ventilation hoods and filters above the cooking equipment.

- comments: previous core violations not corrected from report #2352771 on 12/4/19 not corrected: 47 - cutting boards with deep cut and with dark color. Instructed to refinish the surfaces or replace with new one. Instructed to come into compliance. Priority foundation violation 7-42-090. No citation issued.
