- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed bare hand contact with ready to eat foods by female waitress. Observed her put her hands into a bag of open tortilla chips twice and then placed them in a bowl. Also,male food handler/manager handling cooked tortilla shells from grill and putting cilantro,cheese and onions on top of foods. Must provide gloves or utensils for handling ready to eat foods. Appx. 25 or more rat droppings on floor in between clutter near hot water tanks. Also. A dead mouse on floor behind gaming machine. Must remove droppings. Clean and sanitize areas.
