- comments: observed no clean-up policy procedure and items for vomiting and diarrhea must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: observed thermometer in milk cooler inoperable must replace and maintain.
38. Insects, rodents, & animals not present
- comments: observed an infestation of appx. 500 or more mice and rat droppings on dogfood shelf,dog bowls,candy suckers,bread,can goods,spaghetti sauce,coffee,snacks,chip display boxes,paper goods,water display,rear storage area-along wall bases next to mop sink,reach in cooler & freezer,next to and in between storage bins,on floor next to large light fixture on floor under storage shelves in between clutter,under stairs leading to 2nd fl. Scattered on floor under most shelving units on sales floor. Also. A dead rodent hanging out from bait box behind pop cooler on sales floor. Must remove droppings and dead rodent clean and sanitize areas. Must have a pest control operator to service premises on or after 10/20/22. (priority 7-38-020(a)(citation issued)

- comments: observed food storage items and bins stored on floor throughout rear storage area must elevate articles off of floor 6".

- comments: must clean debris & dust build up from display shelves on sales floor,service counter cashier booths, interior of reach in coolers & freezers.

- comments: must repair or replace some damaged floor tiles on sales floor and womens toilet room around floor drain.
