- comments: observed employee drinking from open coca cola bottle in rear prep area. Instructed manager to provide closed lid containers to prevent contamination of food and food contact surfaces.

- comments: observed no consumer advisory and disclosure/reminder on dine-in menu, take-out menu, and menu board. Instructed manager to provide consumer advisory and disclosure/reminder on all menus. Priority foundation 7-38-005

- comments: observed food items stored on floor in prep area, walk-in cooler, walk-in freezer and storage areas. Instructed manager to elevate all food items six inches off floor.

- comments: observed cut plastic jug used as dispensing utensil inside large pot of beans in walk-in cooler. Instructed manager to provide proper long handle dispensing utensils to prevent contamination.

- comments: observed single service articles not inverted in prep area. Instructed manager to invert all single service articles to protect from contamination.

- comments: observed several pots too large to submerge in 3-compartment sink. Instructed manager to provide alternative method to wash, rinse, and sanitize all large food equipment and utensils.

- comments: observed an excessive amount of grease buildup and food debris on grill tables, prep tables, steam table, fryer cabinets, wheel bases, pipes of cooking equipment, walk in cooler fan guard covers & shelving units, interior panel of ice machine, service counter shelving units. Instructed manager to clean and maintain.

- comments: observed plastic wrapped around faucet of 3-compartment sink in rear prep area. Instructed manager to repair and maintain.
