- comments: 3-501. 16(a2, & b) found large pot of boiled tomatoes and jalapenos for salsa at improper temperature 78 degrees. Product was sitting on burner with flame off all night. Instructed manager to discard product and properly cool and refrigerate future pots. Product was discarded and denatured at this time total value $70. 00 40 pounds priority 7-38-005 citation issued

- comments: observed mice activity in rear storage area. Approx 40-50 droppings. Observed 1 live roach in rear prep area. Instructed manager to clean and contact pest control for a complete service. Priority foundation 7-38-020 (a) citation issued

Instructed manager to provide an approved method to wash, rinse, and sanitize large pots. #55 observed floor tiles damaged and cracked in kitchen. Intructed manager to replace and maintain. #56 observed light shields missing in dish washing area, meat prep area, and utensil storage area. Instructed manager to replace missing light shields and maintain. The following in need of correcting and maintaining.
