10. Adequate handwashing sinks properly supplied and accessible
- comments: noted no handwashing sink next to the 'meat department' three compartment sink located in the dry goods area behind the meat department. Instructed management that handwashing sinks must be located in warewashing areas to be easily accessible for employees. Priority foundation 7-38-030(c).

22. Proper cold holding temperatures
- comments: found cold tcs foods at improper cold holding temperatures inside the taqueria walk-in cooler. Raw chicken, raw pork, tripe, flautas, and cooked pork found at temperatures ranging from 48f to 56f. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued. Food discarded.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: equipment not maintaining proper temperature. Noted temperature of the taqueria walk-in cooler to be improper at 56. 0 f. Unit tagged and held for inspection by the chicago department of public health and may not be used until re-inspected by cdph and maintaining proper temperature of 41f and below. Priority violation 7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: observed no ambient air thermometers inside the hot holding display cases in the taqueria. Instructed to provide conspicuous and easily readable thermometers inside all hot-holding units to properly monitor the ambient air temperature of equipment.

38. Insects, rodents, & animals not present
- comments: observed over 125 mouse droppings on the floor near the rear recieving garage door, on and underneath pallets and on shelves throughout the rear dry goods storage area, inside the boiler room near the the deli department, and behind the barrels of pork fat near the taqueria. Also observed approximately 6 live flies in the employee lounge area. Management instructed to clean and remove all rodent droppings from all affected areas and contact pest control to eliminate all pest activity from the premises. Priority foundation 7-38-020(a). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: found no covered waste receptacle inside the womens washroom. Must provide.

55. Physical facilities installed, maintained & clean
- comments: found dirt and debris on the floors throughout the rear dry goods storage areas and inside the meat walk-in freezer. Instructed to clean and maintain.

- comments: must reorganize and remove any unnecessary articles from the 2nd floor 'seasonal' storage area.

56. Adequate ventilation & lighting; designated areas used
- comments: found heavy dust build up on the fan guards inside the produce, dairy, meat department, seafood, and taqueria walk-in coolers. Must clean and maintain.
