Chicago, 60601 Illinois, United States

Inspectors found several significant food safety violations during the inspection. A high-temperature dish machine was sanitizing utensils at 110°F instead of the required 180°F final rinse, and it was tagged and held for inspection. A prep cooler was found unplugged and holding temperature-controlled foods above the safe cold-holding limit of 41°F, including guacamole, pico de gallo, shredded lettuce, beans, raw fish, and other items. About 42.5 pounds of food were discarded, and citations were issued.

Inspectors also found significant pest issues, including about 50 or more live roaches in cooking and prep areas and about 40 or more live small flies in prep, dishwashing, and storage areas. A broken front door sweep could allow insects to enter. Another significant violation was issued for excessive trash, food, and debris around the shared garbage compactor area.

Additional issues included unlabeled bulk food containers, utensils not stored properly, damaged or worn equipment parts, rusted shelving, grease and food debris buildup on equipment, floors, walls, drains, hood filters, and cooler areas. Inspectors also noted that the rear prep and dishwashing areas had no working air conditioning and temperatures above 90°F.
