1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with a city of chicago certificate. Instructed to have a certified food manager with the city of chicago certificate on site at all times.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site during the inspection while the temperature control for safety foods (yogurts, ice cream, cooked corn etc. ) are prepared and served to customers. Instructed a city of chicago certified food manager must be on site at all times foods while (tcs foods) are being handled prepared and served. Priority foundation violation 7-38-012, citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed some debris accumulation encrusted on the interior of the ice machine. Must remove debris, detail clean inside ice machine/have serviced and maintain ice machine at all times.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed stained cutting boards in the prep area. Instructed to resurface or replace the cutting boards and maintain.

49. Non-food/food contact surfaces clean
- comments: instructed to detail clean the interiors of kitchen cabinets,prep tables, food storage containers,interior of cooler at rear dishwashing area with food spillage.

50. Hot & cold water available; adequate pressure
- comments: observed no hot running water at handwashing sinks and 3-compartment sink. Water temperature at (55. 3f-60. 9f). Instructed management to provide hot running water above 110f at the 3 compartment sink and minimum 100f at all hand washing sinks and maintain at all times. Priority violation 7-38-030(c), citation issued.

- comments: backflow prevention device not located at utility mop sink. Must install so backflow prevention device may be located to be serviced and maintained.

- comments: previous core violations not corrected from inspection report#2521838 dated:06/23/2021 #48--observed missing sink stoppers for the dishwashing 3-compartment sink. Instructed to provide and maintain 3 adequate working sink stoppers. #51--observed no food preparation sink located at front prep area where fresh fruit squeezed juices,milk shakes,nachos and corn are made to order. Must install and maintain. #53--observed no covered waste receptacle provided in the unisex washroom. Must provide and maintain. #56--observed mechanical ventilation in washroom not working. Must repair and maintain adequate ventilation at all times. Instructed to correct noted violations and maintain as required. Priority foundation violation 7-42-090,citation issued.
