- comments: person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued.

2. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager on site during the inspection while tcs foods were being prepared and served. Ie:cooked eggs at 38. 5f. Priority foundation violation 7-38-012 citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee health policy on site for all employees. Must provide a written employee health policy verified by all employees. Priority foundation violation 7-38-010 citation issued.

- comments: observed raw animal origin chicken in the cooks line two door cooler stored in the upper portion of the cooler directly next to ready to eat sliced cheese then moved to a different cooler and then stored directly above ready to eat lasagna. Reviewed proper food protection and separating raw animal origin foods with ready to eat foods. Priority violation 7-38-005 citation issued.

- comments: chemical dish machine not properly dispensing chlorine sanitizer after several attempts. 0 ppm chlorine noted. No dishes being washed during the inspection. No citation issued. Priority violation 7-38-025. Machine tagged held for inspection and must not use until repaired and properly dispenses sanitizer. C. D. P. H. Contacted for tag removal.

- comments: observed the interior of the ice machines upper water reservoir with black and pink slime substance on ice for consumption contact surfaces. Instructed to remove slime substance, clean and sanitize. Priority foundation violation 7-38-005 citation issued.

- comments: observed prepared, ready to eat tcs foods such as cooked eggs, cooked chick, sliced cheeses, cooked lasagna, cooked pasta, cooked roast beef, bacon, alfredo sauce, chicken soup, not properly date labeled with an expiration date. Foods either with no labels or labeled with the prep date only. All foods discarded. Priority foundation violation 7-38-005 citation issued.

and woke up not feeling well. My lower and upper abdomen were extremely swollen and I had horrible cramping as the day progressed I felt worse and worse and my stomach was really messed up.

That evening I opened another container of Fairlife and it had the same odd after taste. I'll be honest because I've never had an issue I still didn't think this was the cause of my issue, in fact I thought my taste was off!

I spent the next few days staying away from straight milk but was using in my coffee and my lattes but was insanely bloated. My husband and I were thinking something was horribly wrong because I looked several months pregnant and had to wear leggings to work Friday. The whole situation was extremely unusual.

My daughter had a cookie and asked for some milk. Upon taking a sip she asked if if it was a different milk and I knew then that it wasn't my tastebuds it was the milk. She said she too felt it had a strange taste... luckily she only had a sip.

I've been absolutely miserable. My clothes don't fit due to bloating, I still have cramps and diarrhea. Both containers were good until June and are from batch 26-207. My husband suggested I check for recalls, come to find that the company has had hundreds of people get sick from this batch and denies issuesz I also learned that they have a lawsuit in regards to animal cruelty which is good to know.
