- comments: observed no hand washing signs at hand washing sinks in prep, dish washing and washroom areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed hot water at hand washing sinks in men washroom(125. 9f) and women washroom(125. 6f) above maximum temperature of 120f. Instructed manager to maintain hot water 100-120f at all hand washing sinks. Priority foundation 7-38-030(c) citation issued

- comments: observed the following cold tcs food stored at improper temperature in prep coolers:3 dozen raw eggs (64f), one pack of canadian bacon (63. 2f), 3 packs of folded eggs (63. 1f), 2 containers of liquid eggs (63f), 5 containers of raw hamburger patties (61. 2f), 3 packs of cheese (49. 5f). Instructed manager to discard approximately $81 and 30lbs of product. All cold tcs food must maintain 41f or below during preparation, display and service. Priority 7-38-005 citation issued

- comments: observed two drawer prep cooler (62. 8f) and prep cooler under serving line (49. 5f) not maintaining proper temperature. Instructed manager to repair and maintain coolers at 41f or below. Coolers are tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued

- comments: observed no thermometers in prep coolers. Instructed manager to provide thermometers inside all coolers.

- comments: observed single service articles stored on floor in rear drive thru area. Instructed manager to elevate all single service articles six inches off floor.

- comments: observed rusty surfaces on shelving units in prep coolers in beverage area. Observed damaged gasket on prep cooler door under serving line. Instructed manager to repair/replace and maintain.

- comments: observed accumulated spills on interior and exterior surfaces of beverage machines and coolers. Instructed manager to clean and maintain.

- comments: observed floor tiles in need of grouting in rear storage area. Instructed manager re-grout floor tiles and maintain.
