10. Adequate handwashing sinks properly supplied and accessible
- comments: front prep area hand wash sink not draining, water leaking on kitchen's floor. Instructed manager to repair hand wash sinks and maintain. Priority foundation violation 7-38-030(c).

- comments: observed at the hand-sink in the prep area. Informed person in charge that paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located infront prep with a build-up of black slime substance, dripping onto ice cubes stored at below ice bin. Ice is used for human consumption. Instructed to clean,sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

22. Proper cold holding temperatures
- comments: noted internal temperatures of the following time/temperature control for safety food items inside the food display cooler to be improper: salmon 46. 8f and tuna at 45. 6f, fish roe at 46. 3f. Observed improper cold holding in rear prep of eggs at 56. 6f and crab rangoon at 63. 5f. Manager immediately and voluntarily disposed of 15 lbs of products worth $250. 00 through denaturing process. Priority violation #7-38-005. Citation issued.

- comments: noted improper cold-hold temperature of 56f in cold holding unit in rear prep informed all refrigerator units shall maintain a proper temperature of 41f and below. Refrigerator unit must not be used until reinspected by chicago department of public health (cdph). Priority violation #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approximately over 600-700 rat and mouse dropping throughout facility in the following areas: 150 in dining room, 100 in front prep area near cold holding unit, and stand-up refer unit. 450 droppings throughout the entire basement storage areas. Instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

- comments: observed over 150 dead roaches throughout basment storage areas, instructed to remove dead roaches and maintain

- comments: observed food stored on the floor in stairway hall leading to basment storage. In the walk-in cooler and freezer, prep and storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: noted one of the toilets out of order due to entire door broken off. Facility obtains a liquor consumption on premises. Instructed to repair. Priority foundation violation #7-38-030(c). Citation issued.

- comments: observed wall base missing throughout entire facility, which includes front prep, dining room, rear prep and entire basment storage area. Instructed to install wall base. Noted must clean debris build up from floor & drains under, around,along wall in dry food & paper storage area and bar. Instructed to install wall base.
