- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation. Priority foundation

- comments: observed no chicago food sanitation manager on site or valid certficate posted when tcs food (chicken, fish, beef) prepared and served. A chicago food sanitation manager must be on site and valid certificate posted at all times tcs food prepared and served. Priority foundation citation issued
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand washing sinks not accessible for hand washing, food handler rinsing shrimp in front hand sink and can of rustoleum and nails inside basin of rear hand washing sink. Instructed manager to remove all items from hand washing sinks. Priority foundation 7-38-030(c) citation issued.

- comments: observed cold tcs food stored at improper temperature inside walk-in cooler, 6 pans of raw chicken 53. 9f-55. 7f, raw fish 52. 1f, 2 containers of tzatziki sauce, 1 container of sour cream, 13 packs of cheese 54. 3f-57. 9f. Instructed manager to discard 150lbs and $300 of product. All cold tcs food must maintain 41f or below during storage, preparation, display and service. Priority 7-38-005 citation issued

- comments: observed food items stored on floor in rear storage, walk-in cooler and walk-in freezer. Instructed manager to elevate all food items six inches off floor.

- comments: observed raw wood surfaces in prep and storage areas. Instructed manager to seal all raw wood surfaces to make smooth and easily cleanable.

- comments: previous core violations not corrected from report #2615765 on 4-14-25:35- raw chicken not properly thawing, sitting in 2 of the 3-compartment sink. Instructed to properly thaw under cold running water, in the cooler or in the microwave. Maintain proper thawing methods. 41-: wiping cloths not properly stored when not in use. Instructed manager to store in sanitizing solution (must be at the required ppm) when not in use. Maintain same. 49-the interior of the ice machine with a pinkish substance on the white panel. Instructed to clean and maintain. 51- leak at the base of the hot water lever handle on the 1-compartment sink and at the middle lever handle on the 3-compartment sink. Instructed to fix all leaks and maintain good plumbing. 53-interior of toilet and the toilet room with stains and debris. Instructed to clean and maintain toilet/toilet room. 55- broken/missing floor tiles inside of the walk in cooler. Instructed to replace/provide and maintain. 56- ventilation filters with dirt, grease and dust. Instructed to clean and maintain filters. 56-employees clothing/shoes stored in the food prep area. Instructed to designate a place for proper storing of personal belongings and maintain. Priority foundation 7-42-090 citation issued
