- comments: missing thermometers in all cooling units on the premises. Instructed to provide and maintain.

- comments: observed evidence of pest activity on site. . . Noted approximately 12 small black live flies fling throughout front and rear prep areas and on exterior equipment surfaces. Approximately 100 small rodent droppings on the premises in the following areas. . . - comments: observed pest infestation. Approximately over 1,000 mice droppings throughout the facility. The following areas were observed with mice droppings on the floor:in the inside of the walk-in cooler with produce stored inside, front service area by the entrance, in the box disposal/ packaging area, surrounding the aquariums with live fish across the meat/seafood prep area, under the food prep tables in the meat/seafood prep area, in the mop sink area, outside the toilet rooms, near the green picking area in the shopping area, the electrical closet at the front, in the office, behind cold holding units in the shopping aisle, and under the pallets in the basement, behind the front service counter, and in the shopping aisles espcially where the pummeto, fruits, and vegetables are stored. The following surfaces were observed with mice droppings: on the pallets in the basement, on the shelving units in the shopping aisle especially where the dried mushrooms are stored, on the slicer and the table where the slicer is stored in the meat/sea food prep area, on the ledge of the wall near the green picking area, and on the boxes in the storage area where the portable gas stove is stored, the lower interior of the cold holding unit near the toilet rooms, in the shelving units behind the front service counter. Instructed to eliminate pest activity, clean, sanitize affected areas and recommended contacting pest control for a service. Priority violation 7-38-020(a), citation issued

- comments: observed dried food items such as dried pork, fish, and mollusks stored throughout the shopping ares in the facility in open containers. Instructed put all food items in containers that are smooth and easily cleanable and able to close and prevent contamination from customers.

- comments: observed the facility missing three sink stoppers for the three compartment sink. Instructed to provide adequate sink stoppers and maintain.

- comments: observed build up of scales on the splash guard and on top of the three compartment sink. Also, the display cold holding units in the shopping with build up of food debris on the inside of the equipment. Instructed to clean and maintain saidequiment.

- comments: observed a leak at the pipe under the washing compartment of the three compartment sink. Water profusely leaking into a bucket. Instructed to repair and maintain.

- comments: observed outside garbage lids completely missing on both receptacles. Instructed to provide and maintain dumpster lids, to prevent rodent harborage. Priority foundation 7-38-020(b) citation issued.

- comments: 3-501. 16(a:2,&b) observed cold tcs foods at inadequate cold holding temperatures located in the rear food prep area. Observed about 100 lbs of raw duck at an inadequate cold holding temperature of 65f during the inspection. Instructed to discard and denature said tcs foods and always maintain all cold tcs foods at 41f or below at all times. Priority violation. Citation issued under violation #21.

- comments: 3-302. 12 observed spices out of the original containers and not properly labeled with the product name on the containers. Instructed to label all containers with food products that are out of the original packaging.

Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 4-603. 15 observed cip equipment that can not be submerged in the 3-compartment sink. Instructed to provide a written procedure for washing, rinsing, and sanitizing all cip equipment.

50. Hot & cold water available; adequate pressure
- comments: 5-103. 11(b) observed inadequate hot water at multiple sinks located in the food prep areas. At the beginning of the inspection, hot water was measured as follows: between 90f-94f at both of the 3 compartment sinks, 90f at two of four hand washing sinks, and 78f at the employee toilet room hand washing sink. Instructed always to maintain adequate hot water at all times; hot water must maintain at least 110f at the 3-compartment sinks and maintain at least 100f at all hand washing sinks. The manager adjusted the hot water temperatures, and by the end of the inspection, hot water was maintaining 110f at both of the 3-compartment sinks and 100f at all said hand washing sinks. Priority violation. Citation issued 7-38-030(c).
