3017 年 4 月 20 日 6 点 30 分左右，在全麦面包、瑞士奶酪、梅奥、辣芥末、甜椒、辣椒、橄榄、番茄和燕麦葡萄干饼干上放了一只小火鸡（火鸡在烤架上加热）。去 Target 购物后开始感到头晕、炎热和恶心。赶回家后立即出现腹泻然后呕吐。在过去的 2.5 小时内呕吐了 4 次，腹泻 6 次以上。也有寒意

2017 年 4 月 18 日下午 6 点 20 分左右抵达德克萨斯州基林洛斯大道的消防站潜艇 76542。我订购了芝士牛排潜水艇，然后分别在下午 6 点 30 分和下午 6 点 45 分左右吃完。症状在 1 小时内（晚上 7 点 30 分左右）开始。

晚上 11:30 左右出现最严重的症状-剧烈呕吐导致大量血液。非常严重的头痛，剧烈呕吐导致脸部和上颈部和背部发红和爆裂的血管。恶心和轻微的幻觉。之后去睡觉了，但我醒了很多次才呕吐更多。截至撰写本文时（2017 年 4 月 19 日上午 9:10）仍然恶心、头痛和干胀（干呕）。
