- comments: found no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation 7-38-010. No

- comments: no soap provided at handwashing sink in prep area. Presently employee handling food. Soap was provided by employee after my request. Priority foundation violation:7-38-030(c), citation issued.

- comments: label dry food containers in storage area included basement storage area with common name,also once food is removed from original containers

- comments: observed pest activity on premises. We observed 30 rat droppings at rear storage area under shelves. Over 25 mice droppings at front dining area between two chest freezers. Instructed to remove,clean and sanitize mentioned areas. Contact pest control for a service. Priority foundation violation:7-38-020(a),citation issued

All raw wooden shelving throughout must be painted/sealed or replaced with a smooth, cleanable and non-absobent material.
