5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable
mops and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005.
14. Required records available: shellstock tags, parasite destruction
- comments: observed (1) container of shellfish (oysters) held inside the walk-in-cooler with no shellfish tag/label. Management discarded,denatured shellfish(oysters) during this inspection. Instructed management shellstock tags are required to be attached until container is empty or retagged and thereafter kept on file for 90 days. Priority foundation violation 7-38-005,citation issued. (cos).
39. Contamination prevented during food preparation, storage & display
- comments: observed no splashguard at handwashing sink at rear dishwashing area. Instructed to provide and washable splashguard at left side next to pot/pan storage shelf.

- comments: observed a torn door gasket inside of the walk-in-cooler. Instructed management to replace the torn door gasket.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no irreversible registering temperature indicators or device to measure the utensil surface temperature inside of the high temperature dish machine. Instructed management to provide and maintain. Priority foundation violation 7-38-005,no citation issued.
51. Plumbing installed; proper backflow devices
- comments: observed water leak at drain pipe underneath the 3-compartment sink. Instructed to repair plumbing leak and maintain.
52. Sewage & waste water properly disposed
- comments: observed wastewater backing-up from open site drain pipe underneath the 3-compartment sink when water is filled and draining. Instructed to repair plumbing and maintain. Priority violation 7-38-030 (c),citation issued.
56. Adequate ventilation & lighting; designated areas used
- comments: observed missing (3) light shields for light fixtures in kitchen prep area. Instructed to provide light shields for light fixtures.

- comments: food handler with incomplete proof of food handler training certificate. Instructed to provide certificate for each employee.

- comments: allergen training is required for all employees that have a city of chicago food sanitation certificate. Instructed to complete training and keep certificate on site.
