2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager with valid original certificate on site while preparing and serving ground beef, nacho cheese, and italian beef. Instructed to provide at least 1 cfm on site at all times while operating. Priority foundation violation. #7-38-012. Citation issued.

36. Thermometers provided & accurate
- comments: unable to locate thermometers inside of display cooler or walk-in cooler. Instructed to provide in warmest spot of cooling unit and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rusty prep table in prep area with excessive rust. Instructed to provide smooth and easily cleanable surfaces.

49. Non-food/food contact surfaces clean
- comments: observed blue bunny ice cream freezer used to store nacho cheese and hot peppers with excessive dried food debris throughout, noted all 3 pressure cookers with accumulated food debris on exterior surfaces of equipment instructed to clean to prevent pest harborage.

51. Plumbing installed; proper backflow devices
- comments: observed leaky faucet neck on 3-compartment sink as well as handsink faucet and neck. Instructed to repair and maintain in good condition.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed missing lid on outside receptable during time of the inspection. Instructed to replace and maintain tight fitted in order to prevent pest harborage. Priority foundation violation. #7-38-020 (b). Citation issued.

56. Adequate ventilation & lighting; designated areas used
- comments: observed inadequate lighting in the rear storage area. Instructed to replace burned-out bulbs and maintain adequate lighting.

58. Allergen training as required
- comments: observed no allergen training on site for cfm. Instructed to obtain and maintain on site for review.
