3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: found no employee health policy on site. Instructed managers to provide a verifiable acknowledgement of their responsibility to report information about their health and activities as they relate to diseases that are transmissible through food to the pic. Priority foundation violation. 7-38-010. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea procedure or cleaning materials to clean up said incident on site. The facility must provide procedures and cleaning materials (gloves, mask, disposable mop, disinfectant rated for norovirus) for employees to follow and use when responding to vomiting or diarrheal events. Instructed manager to provide. Priority foundation violation. 7-38-005. Citation issued.

22. Proper cold holding temperatures
- comments: found improper cold holding temperature of time/temperature control for safety foods. Noted chicken at 65f and coleslaw at 56f located in walk-in cooler. Must provide a minimum cold holding temperature of 41f at all times. Instructed manager to discard and denature said tcs foods approx 15 lbs at $50. Priority violation. 7-38-005. Citation issued.

23. Proper date marking and disposition
- comments: observed ready-to-eat, time/temperature control for safety foods (tcs): cooked chicken, instructed facility to provide discard date for any ready-to-eat tcs foods held for over 24 hours. Priority foundation violation. 7-38-005. Citation issued. Chicken discarded at time of inspection. 5 lbs $20

36. Thermometers provided & accurate
- comments: found no thermometers located in any cold holding units. Instructed manager to provide thermometers to all cold holding units.

37. Food properly labeled; original container
- comments: found bulk containers of food(rice etc) stored in the food prep area without common names. Instructed manager to provide common name for all bulk food containers taken out of original container.

38. Insects, rodents, & animals not present
- comments: found a 1/2 inch gap located under the bottom rear alley entrance door. Must provide tight fitting door at all times. Observed the exit door open and the screen door closed. Also noted is a 1/2 inch gap under the screen alley entrance door. Instructed manager to provide tight-fitting doors at all times.

38. Insects, rodents, & animals not present
- comments: found approx. 500 mouse droppings throughout the facility in the following areas: on the floor in the corner of the toilet room, behind fryers and griddle, in a box of butter located in walk in cooler, on the griddle, under the food prep table, in cooking liquid container stored under prep table in food prep area, on top of expediter station, in unused first prep area were single service boxes are located. Instructed manager to contact pest control for service. Must eliminate pest activity and remove all mouse droppings. Instructed to clean, sanitize and maintain said area. Priority violation. 7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: found food equipment stored on floor in basement. Instructed manager to elevate food items six inches off of the floor.

- comments: observed cooking equipment and prep tables filled with food debris. Instrutced to clean sanitize and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed handwashing sink located in rear next to 3 comp sink slow draining. Instructed manager to repair and maintain.

- comments: found an excessive amount of clutter located in the basement. Noted palettes and food stored on the floor. Also noted unnecessary items in the basement for the food service facility. Instructed manager to elevate everything 6 inches off floor must clean sanitize and maintain said area.

56. Adequate ventilation & lighting; designated areas used
- comments: observed filters located over fryers and griddle filled with grease. Instructed manager to clean sanitize and maintain.

57. All food employees have food handler training
- comments: observed that employee do not have proper food handlers certification

- comments: found no proof of allergen training on site. Must provide allergen training for all city of chicago food service managers at all times.
