1. Person in charge present, demonstrates knowledge, and performs duties
- comments: the pic doesn't have a food managers certificate. Must provide and maintain. (priority foundation )

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (eggs,sausage,ham,etc)must be on site at all times. (priority foundation 7-38-012). Certified food manager arrived on site at 10:10 a. M.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed all (4)hand sinks in prep and dish washing area hand drying devices not operable (not dispensing paper towels. Rear dish washing area no paper towels and men's toilet room hand drying device not operable. Paper towels are now provided at all sinks and dispensing towels. (cos)(priority foundation 7-38-030(c)

36. Thermometers provided & accurate
- comments: must provide a thermometer for front prep cooler.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 7-9 live large flies flying around premises and on wall near unsed display unit,5-7 live small flies on wall outside of toilet room and around light fixture in dining area and 10- 12 live ants crawling on floor under rear large prep table. Recommended to have a pest control operator to service premises on or after 7/8/20. (priority foundation 7-38-020(a)

- comments: observed an appx. "1/2-3/4" gap along bottom of front door. Must make door tight fitting.

39. Contamination prevented during food preparation, storage & display
- comments: must store ice cream tubs from floor in walk in freezer.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must repair or replace worn door gasket on 4- drawer prep cooler,cracked glass on ice cream display freezer.

49. Non-food/food contact surfaces clean
- comments: must clean debris build up from prep tables,interior panel of ice machine,ledges of ovens,exterior of cooking equipment,unused display units,counter tops,dipping wells,walk in cooler- shelving units & fan guard covers. Must defrost ice cream display freezers.

- comments: must repair or replace leaky faucet neck at 3- compartment sink, cracked toilet base in men's toilet room.

- comments: must repair or replace damaged wall base in vestibule. Must grout floor under large prep table and in front of coffee station.
