- comments: must provide thermometers for counter top hot holding unit,rear 2- door prep cooler.

- comments: must label repackaged dessrets for sale with proper info given. Must label food storage containers when food is not in original package.

- comments: observed evidence of appx. 75 or more mice droppings in pizza prep area- inside of a bucket on shelving unit,on floor under and around prep tables,radiator and storage shelves,in dry food storage area-next to radiator,along wall bases next to reach in freezers,radiators and chest freezer,men's staff toilet room next to vanity. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 6/10/21. (priroity foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of rear and side doors,torn screen on side door. Must make doors and screen tight fitting.

- comments: observed wet towels on steam table and prep table must store wiping cloths in a sanitizing solution when not in use.

- comments: must invert multi-use utensils on lower shelves of prep tables.

- comments: must remove rust from shelving units in both walk in coolers,foil from pipes and cardboard from 3- door reach in cooler shelving units and lower shelves of prep tables. Must repair or replace missing fan guard cover in 3- door reach in cooler and doors hinges are in poor repair on same,missing door handle on reach in freezer,worn door gaskets on prep cooler and chest freezer in pizza prep area.

- comments: must clean grease and food debris build up on fryer cabinet,prep tables,interior panel of ice machine.

- comments: observed no cold running water at hand washing sink in pizza prep area. Must use additional hand sinks on site for hand washing. Must repair and maintain. (priority 7-38-030(c)(citation issued)

- comments: must provide a lid on garbage can in women's toilet room.
