- comments: observed the rear dish/prep handwashing sink not accessible for use. Sinks faucet was covered with a plastic bag and unable to use. Instructed to make the sink accessible for use, remove bag and provide hot and cold running water that properly drains, soap and paper towels. Priority foundation violation 7-38-030(c) citation issued.

- comments: no paper towels at either hand washing sinks in prep areas. Must provide at all times. Priority foundation violation. See above violation #10 for citation.

- comments: no metal stem thermometer on-site to monitor all food temperatures and foods with time/temperature as a means of control, such as hot pizza slices. Must provide priority foundation violation 7-38-005 citation issued.

- comments: all bulk flour containers must be labeled with the product common name.

- comments: must provide proper sink stoppers for the three compartment sink.

- comments: noted dirty, rusty food encrusted table top can opener. Instructed to clean after each use. ----all bulk flour containers noted with food splatter. Must clean and maintain.

Priority foundation violation.
