- comments: observed no clean up procedure and supplies on the premises for vomit and diarrheal events. Management instructed to provide both. Priority foundation 7-38-005. Citation issued.

- comments: observed a food employee wash his hands at the three compartment sink before entering the front preparation area. Instructed management that employees cannot use food prep, warewashing, or service sinks to wash hands. Priority foundation 7-38-010. Citation issued.

- comments: observed ready-to-eat, tcs food (salsas) held for over 24 hours not date marked to indicate the date in which the food must be discarded or consumed. Management instructed to date mark all ready-to-eat, tcs foods held over 24 hours; food discarded. Priority foundation 7-38-005. Citation issued.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: found a 2-door cooler broken and not maintaining proper temperature; cooler is operating at 50f. Priority violation 7-38-005. Cooler tagged; instructed to contact cdph as noted on tag and to not use cooler until maintaining 41f or below.
38. Insects, rodents, & animals not present
- comments: observed over 15 live small flies on the bag of onions on the floor and surrounding equipment/items inside the middle dry storage room. Instructed management to contact pest control for service to eliminate all pest activity from the premises. Priority foundation 7-38-020(a). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: found personal employee food stored throughout the sliding 2-door cooler in the dishwashing area. Instructed management to store personal food in a designated area within the cooler that is completely separate from the facility's food.

- comments: instructed management to clean the prep tables, inside coolers, and the interior of microwave.

- comments: noted low hot water pressure in the mens washroom. Must repair.

- comments: noted a hole in the wall underneath the mop sink, in the pink wall across from the three door freezer and next to the mens washroom. Instructed management to repair.

Instructed management to remove all items that are unnecessary to the operation, detail clean area, and organize all remaining articles to provide floor accessibility and ensure adequate facility cleaning.

- comments: previous core violations from 2/11/2021 report #2484766 not corrected: violation #37 (3-302. 12) must properly label food storage containers when food is not in the original container. Violation #40 (2-402. 11) observed employees not wearing hair restraints while food is being prepared and served. Must wear a hair restraint at all times food is being prepared and served. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
