22. Proper cold holding temperatures
- comments: 3-501. 16(a:2,&b) observed time/temperature control for safety foods at improper cold holding temperatures located in a prep cooler in the food prep area; about 2 lbs of guacamole at 52f, about 1/2 dozen raw eggs at 50f, about 10 lbs of raw beef at 55f, about 1/2 lb of shredded lettuce at 50f, about 1 lb of raw chicken at 56f, about 1/2 lb of sliced tomatoes at 51f, about 1/4 lb of chorizo and raw pork at 53f, and about 1lb of cheese at 52f. Instructed to discard and denature said tcs foods and always maintain all tcs foods at a proper cold holding temperature of 41f or below at all times. Manager discarded said items during inspection. Approximately $500 discarded. Priority violation. Citation issued 7-38-005.

25. Consumer advisory provided for raw/undercooked food
- comments: 3-603. 11 observed no consumer advisory posted nor a disclosure and reminder on the menu for all foods that can be ordered under cooked or raw (beef, eggs). Instructed to post a consumer advisory and add a disclosure and a reminder on the menu for all items that can be ordered under cooked or raw (beef, eggs). Priority foundation 7-38-005.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: 4-301. 11 observed a prep cooler, with tcs foods inside, at an improper cold holding temperature of 55f during the inspection. Instructed to always maintain all cold holding units at 41f or below at all times. Unit tagged and held for inspection by cdph and may not be used. Priority violation. Citation issued 7-38-005.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 30 live small flies flying around the bar and customer dining areas and about 10 small flies on various areas of the walls and pictures. Instructed to eliminate live small flies activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: 4-101. 17 observed a flat piece of raw wood under the low temperature dish machine. Instructed to remove, all surfaces must be smooth and easily cleanable.

51. Plumbing installed; proper backflow devices
- comments: 5-204. 12 unable to locate the back flow prevention device on the ice machine. Instructed to provide or locate.

55. Physical facilities installed, maintained & clean
- comments: 6-201. 13 observed coving deattached from the wall under the 3 compartment sink. Instructed to repair and make all surfaces smooth and easily cleanable.

- comments: 2-102. 13 observed no food handlers certificates on site. Instructed to meet requirement for all foods handlers (chefs, waiters and bartenders. )

- comments: 2-102. 13 observed the city of chicago food service sanitation managers without the required allergen training certificates. Instructed to meet requirement.
