- comments: 3-501. 16 observed improper cold holding temperatures for tcs foods. Observed the following foods stored in the front food prep area on the counter to serve to customers: fish at 63. 0f, potato salad at 61. 5f, pita with ground beef and pita with spinach and cheese at 78. 8f, and breaded hot dogs at 73. 1f. Management voluntarily discarded approximately 20# of food worth $150. Priority 7-38-005. Citation issued

- comments: 3-502. 11 observed no cdph approved haccp plan on the premises for the preparation of yogurt. Premises prepares and packages yogurt for human consumption. Management instructed to provide the haccp plan to cdph for approval. Priority foundation 7-38-005. No citation issued.

- comments: 3-602. 11 observed prepackaged soups and salads stored in the display cooler without a proper label. Management instructed to include all required information on label such as name and address of manufacturer, ingredients, net contents, allergens, etc.

- comments: 6-501. 112 observed two dead mice on two separate glue boards in the basement storage area and employee washroom. Management instructed to remove all dead rodents from the premises.

Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.
