38. Insects, rodents, & animals not present
- comments: observed approximately 95 small and large rodent droppings scattered on floors under chip racks and in between all shelving units in grocery (prepackaged items) area. Instructed to clean and sanitize affected areas and contact pest control for service. Must eliminate pest activity. Priority foundation violation. 7-38-020 (a). Citation issued.

38. Insects, rodents, & animals not present
- comments: observed incomplete logbook on site during time of inspection. Instructed to provide a service report for current service of facility by pest control professional. Must maintain on site for review upon request. Priority foundation violation. 7-38-020 (c). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed excessive amount of crates used for elevation throughout facility. Instructed to remove and provide smooth and easily cleanable shelving units. Must maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed excessive garbage , cardboard boxes, stacked up trash and unused equipment stored outside near refuse area. Instructed to clean and remove unnecessary items to prevent pest harbor-age. Must maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed inadequate lighting in rear area near 3 compartment sink. Instructed to provide adequate lighting and maintain.

57. All food employees have food handler training
- comments: must provide food handler training certificates for all food handlers and maintain on site for review.
