3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handsink in food prep area not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Food handler obtained hand soap for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in kitchen area with a build-up of pink slime substance, dripping onto ice cubes stored at below ice bin. Ice is used for human consumption. Instructed to clean,sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

- comments: observed no metal stem thermometer on site to monitor the internal food temperature of tcs foods (rice, chicken, beef) cooked, delivered and served. Facility must obtain and maintain proper thermometers. Priority foundation violation #7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on the premises. Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection reports. Priority foundation 7-38-020(c). Citation issued.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent infestation of over 100 rodent droppings and over 50 live roaches throughout facility. Approximately 100 rat droppings on the floor of the upstairs unused sushi area, scattered throughout the basement and under stairs of rear basement door, corners of rooms in the basement, corner in the bar, in the hallway under dry storage shelf. 10 mouse droppings on top of boxes in the corner of unused sushi area. Also observed over 50 live roaches throughout the kitchen and bar, including bar shelves, in between prep cooler adjacent to kitchen entrance, hood line shelves, dish wash storage rack, dry good storage rack, underneath one door upright cooler, between and around most shelving units and equipment. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority violation #7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed boxes of food stored on the floor in the walk-in cooler. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed wet wiping cloths on several prep surface areas. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment & food. Must maintain

- comments: observed torn door gaskets at all cookline coolers. Instructed to replace torn gasket and remove tape and replace with smooth, easily cleanable surface.

- comments: observed missing and damaged floor tiles behind the bar area. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed an accumulation of dirt/debris buildup on the women toilet room exhaust ventilation and the vent in the hallway above the dry storage shelf. Must detail clean and maintain.

- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

58. Allergen training as required
- comments: observed the city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate.
