2. City of chicago food service sanitation certificate
- comments: the person in charge does not have a city certification or documentation of pending city certificate at the time of inspection while raw eggs is being cooked and served on-site. No other original and valid city of chicago food service sanitation certificate was posted nor provided on-site for the present manager on duty. No valid doccumentation on premises. Instructed to provide a least one certified food manager on site when food is cooked and served on site. - new owners/business managers arrived at approximately 9:05 am with no proof of duccumentaion of city manger or pending city manger certificate. Priority foundation violation 7-38-012 citation issued.

8. Hands clean & properly washed
- comments: food employee switched from preparing raw eggs to ready-to-eat foods (toasted sandwich bread) without washing hands and changing gloves between tasks. Food employee must practive good hygiene by properly washing hands when changing tasks. Discussed proper hand washing with employee and managers on site. Priority violation 7-38-010 citation issued.

9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: food employee used bare hands to handle ready to eat foods (toasted sandwich bread). Food employees shall use suitable utensils such as deli tissue, spatulas/tongs, dispensing equipment or single use gloves when handling ready to eat foods. Discussed importance of wearing gloves when handling ready to eat foods with employee and manager. Priority violation 7-38-010 citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: a sign or poster that notifies food employees to wash their hands is not provided at handwashing sink in back prep-area and all gender restroom used by food employees. Provide a clearly visible handwashing sign at each hand sink.

22. Proper cold holding temperatures
- comments: measured improper cold holding temperature on prep-counter near grill. Raw eggs temperature measured at 51. 2f. Tcs food items were discarded on-site. Approximately 2 pounds at $10 was discarded on site. Priority violation 7-38-005 citation issued.

- comments: dry storage food containers in the kitchen area does not have a lable. Facility must provide.

39. Contamination prevented during food preparation, storage & display
- comments: bowl used as a scoop without a handle in flour container located in basement. Facility must provide a scoop with a handle.

41. Wiping cloths: properly used & stored
- comments: in-use wiping cloths are not stored properly. Found on prep-counter kitchen and near espresso machine. Cloths used to wipe surfaces and equipment shall be held in a proper sanitizer solution. Cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes. In-use cloths must be laundered dailiy.

43. In-use utensils: properly stored
- comments: utensils stored in stagnant water above ice container near front coffee station. Store in-use utensils: 1) in the food with the handle up and out of the food item; 2) stored on a clean and sanitized sirface; 3) in running water; 4) in container of hot water maintained at or above 135f.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: clean utensils stored near the 3 compartment sink and near ice container near coffee station has food contact surface exposed and facing upward. Facility must invert utensils.

51. Plumbing installed; proper backflow devices
- comments: backflow prevention device not found on the espresso machine. Facility must ensure that backflow prevention device is easily located.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: all gender restroom lacks a covered receptacle for disposal of feminine hygiene products. Provide at least one covered trash receptacle in restroom.

55. Physical facilities installed, maintained & clean
- comments: unnecessary items / nonfunctional equipment found in back corner basement. The premises shall be free of items unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used.

- comments: observed missing caulking located at all sink fixtures in the facility. Areas include but is not limited to; exposed handsinks, restroom handsinks, and fountain drink station. Facility must ensure to caulk sink untis adjacent to the walls at all sink fixtures throughout the facility and must ensure all surfaces are smooth, easily cleanable, and non-absorbent.

- comments: missing ceiling tiles and holes in ceiling tiles found in mop sink and kitchen prep-area. Facility must provide ceiling tiles that are enclosed, smooth, easily cleanable, and non-absorbent.

56. Adequate ventilation & lighting; designated areas used
- comments: lighting in the mop closet is found to be insufficient. Light intensity shall be sufficient so that the area, all utensils, equipment, and food items are easily visable.

- comments: previous core violations dated from 11/3/2021. Report #2537673 have not been corrected; #47) must not use milk crates as a means of elevation for stored items in facility. #55) some of ceiling was repaired,however not easily cleanable. Surface must be smooth and cleanable,in basement dry storage area. #56) broken protective shielding at light fixture in prep area, replace. #57) additional food training food handler certificate is required. #58) no allergen training certificates provided on site. Instructed to provide for all certified food service managers priority foundation violation 7-42-090 citation issued.
