    Chicken is associated with more food poisoning outbreaks than any other food according to research by the CDC. Campylobacter and Salmonella bacteria are commonly associated with chicken. Symptoms from the different bacteria can be similar and range from mild to severe including diarrhea, nausea, and vomiting

    - comments: observed food items stored on floor in walk-in coolers and freezers. Instructed manager to elevate all food items six inches off floor.

    - comments: observed single service articles not inverted on prep table under paper towel dispenser in deli prep area. Instructed manager to invert all single service articles to protect from contamination.

    - comments: observed damaged door gaskets on seafood walk-in freezer in rear storage area. Instructed manager to replace and maintain. Observed rust deposits on rolling carts in prep areas and shelving units in bakery display freezer. Instructed manager to repair and maintain.

    - comments: observed food spills and debris on interior and exterior surfaces of cooking equipment, prep coolers, rolling carts, shelving units in prep area. Isntructed manager to clean and maintain.

    - comments: observed leaking faucets on 3-compartment sinks in seafood and meat prep area. Observed right hand washing sink slow draining in women second floor staff washroom. Instructed manager to repair and maintain.

    - comments: during inspection, sliced deli meat, cooked steak, rice, beef stew, chicken wings, prepared sandwiches without any labels instructed to provide proper date marking on alltcs foods prepared on site and stored for more than 24 hours. Person in charge volunteers to discard said items. #7-38-005 citation issued.

    - comments: during inspection, observed no metal stem thermometer for taking food internal temperatures. Instructed to obtain a thermometer and maintain. Citation issued #7-38-005 citation issued.

    - comments: observed approximately 125 small rodent droppings in the following areas during the time of inspection: all shelving areas in the grocery, such as potato chip area, dog food, noodles, beverages, etc. ) scattered on the floor in the utility closet, rear prep area, walk-in storage, and storage closet behind the toilet room. Instructed to remove, clean, and sanitize all said areas. Recommended to contact pest control. Must eliminate activity. Priroity foundation violation. 7-38-020 (a). Citation issued.

    - comments: observed all heavy cooking equipment, display coolers, rolling carts, and shelving units encrusted with grease and dried food debris instructed to clean, sanitize, and maintain.

    - comments: observed unused refrigeration, crates, and unnecessary items stored in the rear of the facility near the outside receptacle area and instructed to remove, organize, and maintain premises.

    - comments: observed no paper towels provided at the hand washing sinks behind the bar, in the basement food prep area, and in the basement washroom.     - comments: observed cross-contamination between raw beef oxtails and raw chicken in the walk-in cooler. Observed both types of raw animal foods in contact with each other and thawing together in the same container and blood and liquid from both meats mixing at the bottom of the container. Management instructed to separate all raw animal foods to prevent cross-contamination. Priority violation 7-38-005. Citation issued.

    - comments: observed no consumer advisory and no reminder statement on the menu for raw or undercooked foods. Management instructed to disclose or describe which foods can be ordered raw or undercooked and link those items to a reminder statement on the menu. Management instructed to add the following sentence before the remidner statement: 'these foods can be ordered raw or undercooked or may contain raw or undercooked ingredients. ' priority foundation violation 7-38-005.

    - comments: must install a splashguard at the exposed hand washing sink next to the steam table.

    - comments: observed no irreversible registering temperature indicator provided on the premises to measure the surface temperature of utensils inside of the high-temperature dish machine. Management instructed to provide. Priority foundation violation 7-38-005. No citation issued

    - comments: clean the exterior of the chest freezer; door handles for the walk-in cooler and reach-in coolers, interior of the prep coolers, prep tables, top of the convection oven, and exterior of the stove.

    - comments: repair the leak at the drain line at the exposed hand washing sink next to the steam table in the 1st floor prep area.

    53. Toilet facilities: properly constructed, supplied, & cleaned
    - comments: observed no self-closing device on the 1st floor employee washroom door. Instructed management that all washroom doors must be completely enclosed and provided with tight fitting and self-closing doors.

    - comments: clean the dust and grease on the ventilation hood and filters above the cooking equipment.

    - comments: observed several food handling employees without a food handler certificate. Management instructed that all food handling employees must show proof of training.

    - comments: previous core violations from report #2577947 on 6/27/23 not corrected: 47 - observed cutting board in rear prep area covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable cutting surfaces. 53 - observed no covered trash receptacle inside the employee's toilet room. Instructed to provide. Priority foundation violation 7-42-090. Citation issued.
