- comments: observed raw animal-origin foods stored improperly with prepared ready-to-eat foods. Raw chicken, raw beef, and raw shrimp are stored uncovered, directly next to and touching cut, prepared vegetables such as cabbage and carrots in the
upper portion of the cooks line two-door cooler. Also noted was raw beef stored on a metal rack shelving inside the walk-in cooler directly above uncovered prepared noodles and uncovered prepared and cut fresh broccoli. Instructed to store separately. Reviewed proper storage of raw and ready-to-eat foods. Priority violation 7-38-005 citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: the menu does not disclose and inform consumers of the specific menu items, such as sushi and other assorted raw fish that are raw or undercooked. Must provide a consumer advisory that discloses and reminds customers of such food on both the printed and virtual menus. Priority foundation violation.
39. Contamination prevented during food preparation, storage & display
- comments: observed approximately 5 lbs of freshly washed, ready-to-eat green onions on a metal pan drying outside on the rear patio under the buildings wooden staircase, not protected from environmental contamination. All discarded. Also noted were prepared foods being stored directly on the floors of the walk-in cooler and walk-in freezer. Rice cooker operating and cooking rice on the floor of the kitchen. Reviewed proper food protection and storage of foods. Priority foundation violation 7-38-005 citation issued.

- comments: all food handlers must wear effective hair restraints.

- comments: all prep area wiping cloths must be held in a container with a sanitizing solution.

- comments: all stored items throughout the rear service sink closet, rear storage room, and front counter must be elevated from the floors.

- comments: torn rubber door gaskets on the cooks line two-door cooler and door repaired with tape. Must replace gaskets.

- comments: the following equipment was found dirty with excessive grease and food debris: all cooks line equipment, the exterior of the dish machine, and the interior and exterior of all refrigeration, freezers, and microwave oven. Exterior of all bulk food storage containers. Must clean and maintain all.

- comments: floors in the following areas with dirt, grease, and food debris: under the wok cooks line, inside the walk-in cooler and freezer, behind the unused ice machine, under all rear bulk stored foods, under the dish machine, throughout the mop sink room and rear dry food storage room. ----walls at the dish machine with dirt and splatter. Must clean and maintain all.

- comments: noted soiled mop head improperly stored in the kitchen. Must store inverted at the utility service sink.
