- comments: no certified food manager present during inspection. Priority foundation. No citation issued. See #2.

2. City of chicago food service sanitation certificate
- comments: no city of chicago certified manager on duty while tcs foods (chicken, beef ribs, pork chops) are being prepared and served. Priority foundation. 7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees. Priority foundation. 7-38-010. No citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit and diarrhea policy or supplies. Instructed to provide necessary items for vomit and diarheal events (gloves, facemask, disposable mop, disinfectant to combat norovirus).

22. Proper cold holding temperatures
- comments: observed tcs foods held at improper temperature in 2 door refrigerator: chicken (46. 6f), pork (45. 3f), rice (47. 8f), milk (50. 0f), eggs (51. 1f), fish (46. 8f). Employees discarded approximately 50 pounds of food products at a cost of $400. Priority. 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed 2 door refrigerator holding improper ambient air temperature of 49. 7f. Refrigerator tagged for re-inspection. All cold holding equipment must hold air temeprature of 41f or below. Priority. 7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: no metal stem thermometer on site to measure internal food temperatures. Must provide working, calibrated, metal stem thermometer. Priority foundation. 7-38-005. Citation issued.

37. Food properly labeled; original container
- comments: observed large bins of flour and sugar in dry storage area not labeled. Any food that is not in original container must be labeled with common name.

- comments: observed rear screen door improperly fitting door frame at the top. Instructed to provide a tight fitting door to prevent pest actvitity inside establishment.

- comments: observed top of chest freezer in extremely poor repair with exposed insulation and torn gaskets. Gasket on door of 2 door refrigertor is torn. Must repair and maintain chest freezer top and refrigerator gasket.

- comments: no chemical sanitizer strips on site. Instructed to provide appropriate chemical test strips. Priority foundation. 7-38-005. Citation issued.

- comments: employee handwashing sink in washroom is slow draining. Must repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: employee washroom door not self-closing. All washroom doors must be self-closing.

55. Physical facilities installed, maintained & clean
- comments: observed vent cover hanging from ceiling above serving line. Must repair and maintain. Observed stained ceiling tiles in lobby of restaurant. Must replace and maintain.

60. Previous core violation corrected
- comments: previous core violations from previous inspection 01/08/2020 not corrected: #47 observed the cutting board in the prep area on the left side with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times. Observed the interior and exterior of the microwave stored in the rear of the back room not clean. Instructed to clean and maintain at all times. #53 observed no covered reeptacle in the employee restroom. Instructed to provide a covered receptacle in the employee restroom. #55 observed missing and damaged floor tiles in rear where the 2 door cooler and chest freezer is stored. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times. #60 premises has no allergen training. Not corrected within 90 day time frame on the inspection report dated for 01/08/2020. Priority foundation. 7-42-090. No citation issued.
