- comments: observed no written employee health policy on the premises. Management instructed to have a signed employee health policy for each employee. Priority foundation 7-38-010. No citation issued.

- comments: observed no clean up procedure or supplies on the premises to respond to vomit and diarrheal events. Management instructed to provide both. Priority foundation 7-38-005. No citation issued.

- comments: observed improper cold holding temperatures of tcs foods inside of the 3 door prep cooler. Observed eggs at 48. 7f, milk at 49. 5f, beef at 54. 4f, pork at 51. 3f, cream cheese at 48. 2f, and ham at 47. 5f. Management voluntarily discarded approximately 45# of food worth $300. - comments: observed packaged flan stored in the prep cooler without a label. Management instructed to label all prepackaged foods with name and address of premises, net contents, and ingredients. Management instructed to properly label all prepackaged foods.

- comments: observed chipped paint and rust on the wire storage racks in the walk-in cooler. Management instructed to remove chipped paint and rust and repaint or replace the shelves.
