Report date: Jun 1, 202222. Proper cold holding temperatures
- comments: found cold tcs foods at improper cold holding temperatures in the 2-door cooler in the prep area, and inside the walk-in cooler. Cooked shrimp, salsa, pico, beef barbacoa, and pork found at temperatures from 46f to 50f inside the 2-door cooler. Large and tightly sealed containers of chicken and shrimp soup, and sealed plastic bags of cooked rice found at temperatures from 48f to 52f inside the walk-in cooler. Instructed management to discard and denature said tcs foods and always maintain all cold holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: observed an incorrect menu consumer advisory for raw or undercooked foods. Instructed to identify such foods by asterisking them to a foot note that states: 'items can be served raw or undercooked or contain undercooked ingredients' and add a statement that says: 'consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions'. Priority foundation 7-38-005.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed the temperature of the 2-door cooler in the prep area to be improper at 46f. Unit tagged and held for inspection by the chicago department of public health and may not be used until re-inspected by cdph and maintaining proper temperature of 41f and below priority violation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed 5 live cockroaches in the rear food prep area around the 3 compartment sink and near the bottom of a storage table. Instructed management to remove all roaches and activity, clean/sanitize affected areas, and to contact pest control for additional service. Priority foundation 7-38-020(a). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed a 1/2 inch gap at the top of the front door. Instructed management to pest proof said door and make tight-fitting.

- comments: noted milk crates being used as shelving and for storage throughout the kitchen. Must remove from premises and provide shelving that is free of unnecessary crevices and designed/constructed to be easily cleanable.

- comments: found cardboard and aluminum foil used as liner inside coolers and on storage tables in the rear prep and storage areas. Instructed to remove the foil/cardboard so surface is smooth, non-absorbent, and easily cleanable.

- comments: noted heavy ice build up inside four of the chest freezers in the rear storage area. Instructed management to defrost, remove ice, clean, and maintain.

- comments: observed cardboard being used as floor mats in the rear prep area. Must remove and provide floor coverings that are a non-absorbent, non-corrosive, smooth, easily cleanable surface.
