High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed white rice (105 F - Hot Holding); cooked beef (115 F - Hot Holding) at kitchen area. As per operator for less than one hour. And rice with beans (128 F - Hot Holding) and cooked beef (120-125 F - Hot Holding) at steam table. As per operator for less than one hour. Operator started to reheating the products. **Corrective Action Taken** **Repeat Violation** **Warning**

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. **Repeat Violation**

High Priority - Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed chlorine sanitizer solution at +200 ppm at three compartment sink. Operator added water to get 100 ppm. **Corrected On-Site** **Warning**

Had the new smokin chicken with the creole pepper sauce around 11:30, I started feeling sick around 6pm. Was so sick and weak called the EMTs around 9:30 pm. Nothing they could do, vomiting and diarrhea can’t keep anything in.  | Symptoms: Nausea, Diarrhea, Vomiting, Weakness

had to run to the bathroom multiple times. I ended up getting chest pains from the convulsions of throwing up so much. I had to be rushed to the emergency room due to pains in my chest and abdominal pain. They did tell me I had food poisoning and possible salmonella from what I ate.  | Symptoms: Vomiting, Stomach Pain, Chest Pain
