- comments: observed no clean-up policy procedure and items for vomiting and diarrhea must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: must label food shakers/food storage containers/bins when food is not in original package.

- comments: observed evidence of appx. 40 or more mice droppings on floor in walk in cooler next to shelving units, on floor behind large on wheel food storage containers,on a baking pan on lower shelf with chemical stored on it. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 2/21/24. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of both front entry doors and rear door must make doors tight fitting to prevent rodent entry.
