28. Toxic substances properly identified, stored, & used
- comments: observed spray bottle with unknown chemical stored in serving area of kitchen without proper common name marking. Must provide proper marking on all chemical containers. Priority foundation violation, 7-38-015

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed reach in cooler not maintaining proper temperature 49. 2f. Instructed manager to repair and maintain cooler at 41f or below. Prep cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed open bait trap located in storage area. Manager removed bait during inspection.

51. Plumbing installed; proper backflow devices
- comments: observed leaky pipe at 3 compartment sink in kitchen. Engineer on site repaired sink at time of inspection.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles in staff/gender neutral and girls restroom. Instructed to provide and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed the outside dumpsters overflowing with garbage. Instructed to make sure all lids stay tight fitting and do not overflow outside dumpsters with garbage. Priority foundation 7-38-020(b).

55. Physical facilities installed, maintained & clean
- comments: observed damaged/cracked walls and peeling paint in kitchen. Instructed manager to repair and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed burned out light bulb under ventilation hood. Instructed to replace and maintain.
