- comments: the following tcs foods at improper cold holding temperatures in walk in cooler: pork 55. 8f, green salsa 57. 2, beef 59. 2f, and cheese at 64. 9f. Also observed large filled garbage can of raw pork unrefrigerated and in prep area at improper temperature off 55. 8f. All above food discarded and denatured. Approx 150lbs $500 value. Instd to maintain cold foods at 41f or below at all times. Citation issued 7-38-005 priority.

- comments: inadequate refrigeration on premises. Walk in cooler unable to maintain proper cold holding temperatures, internal air temperature of 50. 1f. No other refrigeration on site. Instd to have refrigeration at 41f below at all times. Citation issued 7-38-005 priority

- comments: infestation of pests observed on premises. Over 100 large live house flies noted through out premises on walls, food and equipment in prep area. 2 live roaches crawling on storage container in prep area. Numerous dead baby roaches noted inside deep freezers. Over 50 mice droppings noted on floor along wallbase in basement. Over 20 rat droppings noted in rear exterior storage area. Excessive clutter, unnecessary articles causing harborage in outside rear. Instd to remove all clutter and organize area. Instd to remove all flies, roaches, rodent droppings, clean all affected areas and updated pest control service for premises for service. Citation issued priority 7-38-020(a)

- comments: rear double exit door not protected. Approx 1/2 inch gap noted in bottom corner and inbetween doors. Instd to repair door and have tight fitting.

- comments: raw wood surfaces noted on walls, wall coverings, and ceiling beams through out front area. Large opening in ceiling in rear storage area. Instd to seal all raw wood and large opening, so as surface to be smooth, even, and easily cleanable.
