Corrected on site manager voluntarily discarded and denatured food valued at $1000 weighing 80lbs priority violation 7-38-005 citation issued

- comments: observed two cold holding units at improper ambient temperature. Observed two door refrigerator and small two door cold holding unit in first floor rear prep area at improper ambient temperatures ranging between 56. 0-56. 3f. Instructed all cold holding units must be maintained at 41f or below. Corrected on site held for inspection tags placed on two door refrigerator and small two door cold holding unit in first floor rear prep area. Priority violation 7-38-005 citation issued

- comments: observed cracked interior surface of lower ice machine door. Instructed to smoothly repair or replace interior surface of lower ice machine door to prevent contamination of ice. Observed damaged gaskets on both doors of two door refrigerator in rear first floor prep area. Instructed to repair or replace damaged gaskets on both doors of two door refrigerator in first floor rear prep area.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no irreversible temperature indicator for measuring surface temperature of utensils and other dishes. Instructed manager to provide. Priority foundation violation 7-38-005 no citation issued.

- comments: observed leaking faucet at three compartment sink in bar. Instructed to repair leaking faucet at three compartment sink in bar. Observed water draining from drainpipe of ice machine onto floor. Instructed to repair drainpipe of ice machine for proper waste water drainage into floor drain.

- comments: observed no covered receptacle in all gender employee washroom. Instructed to provide covered receptacle for all gender employee washroom for sanitary products.

- comments: observed ice buildup on condensor line in walk in freezer. Instructed to remove ice from condensor line in walk in freezer and maintain.

Observed missing grout in between tiles throughout first floor rear prep area. Instructed to smoothly replace missing grout in between floor tiles in first floor rear prep area near floor drain near two compartment sink and in walkway leading to dining area for ease of cleanability.

- comments: at time of inspection facility provided expired allergen awareness training certificates for employees with city of chicago food service sanitation certificates. Instructed to provide valid proof of allergen training for all employees with city of chicago food service sanitation certificates.
