- comments: 4-101. 19 must not use milk crates as a means of elevation for stored items.

51. Plumbing installed; proper backflow devices
- comments: 5-205. 15 must not use raw wood as a means of elevation for under two-door reach-in cooler. Must provide a non-absorbent material. Excess water inside the two-door reach-in cooler, repair cause. Must indicate on water handles 'hot-cold' inside staff and residents washrooms to able to determine which is hot or cold handle

54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 12 cement ground excess standing water and food liquid spillage under receptacles due to area is uneven and forming puddles of water. Flies around said water and receptacles. Repair cause

55. Physical facilities installed, maintained & clean
- comments: 6-201. 11 broken and missing baseboard wall tile in the kitchen by and behind the ice machine and walk-in cooler. Replaced cement floor uneven with rough surface by the walk-in cooler and throughout the kitchen, surface must be smooth and easily cleanable. Missing wall tiles inside women's staff washroom and nurse washroom(first floor). Broken cement and missing floor tile at mop sink first floor, at same area broken/holes on ceiling. Missing ceiling tiles in the kitchen above the prep table are also broken and heavily stained in the dining area. Replace.
