Replaced cement floor uneven with rough surface by the walk-in cooler and throughout the kitchen, surface must be smooth and easily cleanable. Missing wall tiles inside women's staff washroom and nurse washroom(first floor). Broken cement and missing floor tile at mop sink first floor, at same area broken/holes on ceiling. Missing ceiling tiles in the kitchen above the prep table are also broken and heavily stained in the dining area. Replace.
