1. Person in charge present, demonstrates knowledge, and performs duties
- comments: pic doesn't have a food managers certificate. Must provide and maintain. (priority foundation)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs ready to eat foods are being prepared,handled and served such as (hamburgers,italian sausage,italian beef etc) must be on site at all times. Manager arrived on premises at 2:10 p. M. (priority foundation 7-38-012)

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing sink in dish washing area. Must install a hand sink with hot and cold running water. Must provide and maintain. (priority foundation)

36. Thermometers provided & accurate
- comments: must provide thermometer for beige refrigerator.

- comments: observed evidence of appx. 120 or more mice droppings on floor next to and behind both chest freezer,in between clutter on rear storage shelves,on loose wall base near rear door,along wall in hot water tank room,along walls in rear unused prep area,in between clutter and along walls in both unused walk in cooler,on floor and on pipes in front meter closets,near rear 3- comp sink. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 3/11/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along bottom of rear door and front entry door. Must make doors tight fitting.

- comments: must store wiping cloths in a sanitizing solution when not in use.

- comments: must repair or replace loose lid on rear chest freezer.

- comments: must clean debris build up from fryer cabinets,unused equipment stored in storage areas,some storage racks,prep tables.

As of now, no cases of allergic reactions associated with the consumption of these foods have been reported to MAPAQ.

The affected products are:
- Banh bao pork and egg (Wheat) / Unitary
- Chicken Poke (Eggs and Mustard) / Unitary
- Chicken Yakisoba (Wheat, Molluscs - Oysters) / Unitary

All batches of these products are included in the recall if their labels do not mention the presence of the allergens. These items were sold packaged in bowl-like cardboard containers or wrapped in clear plastic wrap and were sold refrigerated.

People with an allergy to wheat, eggs, shellfish or mustard, or those with celiac disease or another illness induced by the ingestion of gluten are advised not to consume the recalled products. They must return it to the establishment where they bought it.

that are the subject of this warning were offered for sale only at Fleur de Sel located at 601 Rue Principale, Granby, QC, Canada. The issue was discovered during routine checks by the MAPAQ. As of now, no allergic reactions related to the consumption of these products have been reported.

The recalled products include "Filet de Sole Stuffed with Salmon and Shrimp", "Cod Loin Saffron and Chives", and "Basil and Tomato Chicken". These items were sold frozen and vacuum-sealed in clear plastic bags. Specific details such as brand, size/weight, batch, UPC, and best before date were not provided, nor was the amount of recalled products. The products were vacuum-packed in a transparent plastic bag.

Please do not consume this product if you have it at home and return it to the store where it was purchased.
