- comments: 4-601. 11(a) observed a black substance in the interior of the ice machine on the ice maker and the top of the interior. Instructed to clean and sanitize. Priority foundation. Citation issued 7-38-005.

- comments: 3-302. 12 observed some spices out of the original packaging and a container without a label with the product name. Instructed to provide for all foods that are out of the original packaging.

- comments: 6-501. 111 observed evidence of pest activity. Observed about 15 rat droppings located in the hallway storage area that leads to the basement under the storage shelves and boxes. Instructed to eliminate rodent activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 4-502. 13 observed grocery bags being used for food storage in the walk-in cooler, walk-in freezer, reach-in freezers, and 2-door freezer. Instructed to not use grocery bags for food storage and only use food-grade bags or food-grade containers to store all foods.

- comments: 4-601. 11(c) observed accumulated grease between the cooking equipment and on the vents above the wok/fryers. Instructed to clean all.

- comments: 5-205. 15(a) observed the hot water unable to turn on at the hand washing sink in the dish washing area. Instructed to repair and always maintain hand-washing sinks at all times. Priority violation. Citation issued 7-38-030(c).

- comments: 6-303. 11 observed inadequate lighting in the basement and in the hallway storage area that leads to the basement. Instructed to increase lighting.
