- comments: found the following tcs foods held at improper temperatures in front 3 door sushi reach in cooler measured at crab (cooked) at 51f, raw flounder/sea bass at 51. 2f, mayo spicy sauce at 52. 7f, raw salmon at 50f, tobiko (fish eggs) measured between 51. 4 - 53. 9f, cooked shrimp at 55. 2f, raw octopus at 54. 8, raw eel 53. 9-55. 5f, prepared shitake mushrooms between 55. 5-56. 3, and prepared gourd shavings between 51. 4-63. 8f. All tcs foods were discarded on site. Priority violation 7-38-005 citation issued
33. Proper cooling methods used; adequate equipment for temperature control
- comments: founds sushi reach-in cooler not maintained, ambient air temperature at 57. 7 f. Facility must repair and ensure a temperature of 41f and below. Priority violation 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed pest infestation in food prep and storage area: found more than 25 live roaches in prep areas reach-in coolers and rear prep area floor and walls. Also observed were more than 50 rat droppings on the basement floor along walls and corners. Instructed must remove all roach activity and rat droppings on-premises. Priority violation 7-38-020(a) citation issued.

- comments: excessive grease buildup in/around hood system above cookline. Facility must clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed large gaps/holes throughout walls and ceilings throughout basement covered with wire mesh. Facility must properly seal and provide smooth and easily cleanable surfaces throughout.
57. All food employees have food handler training
- comments: expired certitfed food handler certificates found on site. Facility must ensure all food handlers have updated food handler certificates.
58. Allergen training as required
