2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods were prepared and served at the time of inspection. Certified food manager arrived at facility 30 minutes after inspection started. Instructed person in charge that a certified manager with a current city of chicago certificate must be on premises at all times when time and temperature control for safety(tcs) foods are being prepared, handled and/or served. Priority foundation violation #7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees. Priority foundation violation #7-38-010. No citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean up policy and supplies to properly handle bodily fluid spills on site. Priority foundation violation #7-38-005. No citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink in food prep area not accessible. Basin was filled with utensils and a plastic food containers. Instructed that sink must be accessible and maintained at all times. The items were removed during the inspection. Cos priority foundation #7-38-030(c). Citation issued.

36. Thermometers provided & accurate
- comments: observed no metal stem thermometer on premises. Instructed manager must provide and calibrate a metal stem thermometer for food handlers and keep on premises. Priority foundation 7-38-005. Citation issued.

51. Plumbing installed; proper backflow devices
- comments: observed a slow drain at the hand sink in the food prep/dish area. Instructed to repair.

55. Physical facilities installed, maintained & clean
- comments: observed missing and damaged floor tiles in the food prep area and dish area. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

60. Previous core violation corrected
- comments: observed previous core violation #51(-compartment sink missing stoppers (x2). Instructed manager must provide stoppers for 3-compartment sink), #57(no food handlers certificates on premises. Instructed manager food handlers must have certificates with names of employees on premises) #58(no food allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates on premises) not corrected within 90 day time frame on the inspection report #2293714 dated for 06/17/19. Priority foundation 7-42-090 no citation issued.
