1. Person in charge present, demonstrates knowledge, and performs duties
- comments: no person in charge was present at the time of inspection. Priority foundation violation. No citation was issued.

2. City of chicago food service sanitation certificate
- comments: noted no certified food manager on duty while tcs foods (chicken, hamburgers, salads etc. ) were being prepared and served. Instructed management that a certified food manager must be present at all times tcs foods are prepared and served. Also, the person in charge must have a city of chicago certificate by the next inspection (upon being inspected) priority foundation violation 7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: the person in charge is unable to provide health employee policy instructed to provide and maintain. Priority foundation violation 7-38-010. No citation was issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: management is unable to provide a procedure for cleaning up vomiting and diarrhea. Instructed to have one, including the chemicals and supplies needed. Priority foundation #7-38-005. No citation issued

36. Thermometers provided & accurate
- comments: noted no thermometers inside refrigeration unit conspicuously posted to monitor the ambient temperature of equipment. Instructed to equip all refrigeration units with accurate and working thermometer

38. Insects, rodents, & animals not present
- comments: noted over 50 flying insects on walls, shelves, ceiling, and equipment at the dishwashing area. Instructed to detail, clean, and sanitize affected areas, eliminating pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation #7-38-020(a).

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted torn rubber gasket inside the prep service cooler door by the take-out counter. Instructed to repair and maintain and/or replace.

- comments: noted ice build-up inside the walk-in cooler on the floor and shelves. Instructed to repair and maintain the condensation units.

- comments: noted the following:- missing tiles on the floor by the ice machine at the kitchen prep area, dirty standing water inside the missing tiles walls behind the sprayer sink, and dishwashing machine with black and mold-like substance. Instructed to repair floor, maintain dry and detail clean walls and maintain.

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and maintain floors throughout, especially the kitchen, dishwashing area, and bars. Must also clean and maintain all floor drains.

57. All food employees have food handler training
- comments: noted employees at work with no food handler certificates. Instructed management that all employees are required to have the food handler certificates on-site at all times and one month (30 days) grace period for new hires

58. Allergen training as required
- comments: noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.
